Wrote the song about people that struggle with addiction to cocaine and that is where the lyric When the Blade comes down/out came from. The lyric - the ropes been twisted too tight - is about the addiction twisting you up and not letting you go. Although it was written at the time about cocaine, it could mean any drug, or other addiction that cuts you down.
