It's sometimes easier to say "I'm Fine" when you are asked how you are. Most of the time, that's a small talk question and nobody really wants to know the answer. When we, humans, struggle with things like physical pain, financial stress, emotional issues, doubts in faith, it makes things hard. But we have to push through, carry on, in order to live.
